The cardiovascular SCN has met three times since it was established in the spring of 2013. During this time it has undertaken a stakeholder event to start to focus on its detailed work plan priorities for 2014/15 based around the domains of the NHS Outcomes Framework.

Domain 1: Preventing people from dying prematurely

  • Acute Kidney Injury (AKI) - This project is aimed at earlier detection and management of AKI.  It will include the development and implementation of an e-alert system in all pathology departments throughout Wessex; development of management guidelines and a Wessex wide primary care education programme.
  • Diagnostic pathways for cardiac, stroke and diabetes - This will involve timely and appropriate access to diagnostic services for patients through the development of effective, high quality care pathways.
  • Health Checks - Although this is a national programme which aims to tackle avoidable deaths, disability and reduce health inequalities, there is wide variation in uptake throughout Wessex.  This project aims to improve uptake throughout Wessex and thereby improve individual health outcomes and experiences related to cardiovascular conditions.

Domain 3: Helping people recover from episodes of ill health or following injury

  • Rehabilitation - The aim of this project is to develop an out of hospital minimum service specification to ensure a consistent approach to rehabilitation throughout Wessex.  The cardiovascular SCN is leading this piece of work with the intention that that it can be adopted by the MHDN and Cancer SCNs.
